does a day 21 progestrone test have to be on day 21?

not strictly but it needs to be after ovulation.  Some clinics make you do OPK's detect ovulation and then do it 5-7 days afterwards. d21-22 would be fine I guess if you know how many days after ovluation they can plot it

A serum progersterone test should ideally be done 7 days after you ovulate, so no it should not always be done on cd21. It should be specific to the individual.

The problem with this of course is if you don't know when/if you ovulate. Its generally best to count back 7 days before the last day of your average cycle and have the test done then e.g if your cycle is usually 28 days then you would have the test on cd21, if your cycle is on average 30 days you would have the test done on cd23.

im on cd 13 now and i am testing for the surge as yet nothing, i know i have follies as i had a saline scan on day 9

maybe i will talk to the clinic

Hi

Progesterone peaks at 7dpo which is ideally when it should be tested.  Having tested on cd21 assumes ovulation on cd14.

If you use the search facility you'll find loads and loads of threads discussing this.

They usually like follicles to be minimum of around 18mm before rupturing so it could just be that although you're on cd13, your follicles aren't quite ripe yet....hopefully you'll get the surge in next day or so which will mean ovulation around 36 hours later.

Hi all

So glad we are on this topic as I am sooooo confused.  I am going to be seeing a fertility specialist on the 9th of June and need to havd cd21 bloods done which i believe is the progesterone test - anyways I have been using the opk and for 2 months in a row I am detecting my LH surge on CD 17 meaning I am ovulating up to 36 hours thereafter meaning ovulating around CD18/19 - am i right?

Now I think I am 28 day cycle as without fail I get my period on cd28 so I decided to have my cd21 test on cd21 - which was on Friday ( I used my full blood bleeding day as day 1) and I got the results today and the nurse has told me that by my hormones in the blood she has picked up that I hadnt ovulated yet and to go back this thursday - when I am expecting to have my period??? as she thinks I am a 35 day cycle - how can that be if I bleed every 28 days??? also surely the opk test's wouldnt be making it up that I am surging?
I still have next cycle to play around with these test to try get them on or around the 21 day - when do you suggest I get them done.  Now I dont even know when I could be ovulating to BMS!!! so now I guess I have to BMS like every second day as I am so confused now. - any advice please!!!

Hiya - from what you say it sounds to me like you may want to go in a little earlier not later! That way you can always go back a few days later too to retest if needs be...I'd use the OPK, get a dark line and then go in a day later...then go from there ;)

I do know that it's possible to get a + on an OPK and not actually ovulate. Your body gears up for ovulation and yet it doesn't happen.

But let's say you do OV on CD19 then that would mean going for your Progesterone test around cd26 for the best result. For a 28 day cycle CD19 is late to ovulate - but not everyone has a text book cycle. Next time you get a positive reading on your OPK I'd book in for a test about 8 days later.

Arrgghhh....just lost a really long reply...ok, now I'll try and remember what I said ;)


...progesterone peaks at 7dpo so having tested on cd21 assumes ovulation on cd14.  However, a luteal phase can be anywhere between 10 - 17/18 days and still be classed as normal...complete myth that always 14 days from ovulation to AF.

If you're getting a +ve OPK on cd17 then this means you should be ovulating around cd18/19 (approx 36hrs after the LH surge).  So ideally you should be getting your progesterone tested on cd25/26 even if your AF does arrive a couple of days later.

Do you know what the actual progesterone level was from your last test ?  Clinics usually look for minimum 30 nmol/l to indicate ovulation occurred.  If your levels were quite a bit lower than this then this could indicate no ovulation...or poor ovulation ie the egg was too immature or too mature when released so not great progesterone levels.  If your progesterone was "borderline" ie mid-high 20's then this could possibly mean that you were tested a bit early/late.

Personally I'd think it would be interesting to know what your previous level was and what it is when tested 7dpo...and if your levels are showing higher at 7dpo (even if late ovulation and AF arrives few days later) as then you can mention all this to your consultant as it could possibly mean your luteal phase is at the shorter end of what's "normal".

Obviously I am no way medically qualified and this is just info I've picked up from reading but also what I've been told by those who are medically qualified ie fertility consultant (x 2 !)

Thanks you  have told me so many times about when the best day to test is! but i just get soooo confused!  today is my actual cd25 so I should of gone to clinic today to get them cd21 day tests dont presume I am ovulating on cd18/19

I did not get my result according to the nurse she said that it looked like I was just about to ovulate???? does this mean anything to you?

I am going to try get a docs appointment tomorrow to get bloods done( tomorrow is Cd26) do you think I should do that??

Can my cycle be 35 days long like the nurse said?? and me bleed at 28 days?

As I say, I'm not medically qualified but I have absolutely no idea what the nurse meant when she said by your results then you're about to ovulate ??!!?  Progesterone is released from the corpus luteum which is the area of the follicle where the egg ruptures....so if you had progesterone then you must've ovulated.  Yes, you can have lower levels of progesterone earlier in cycle but I'm not sure exactly how she'd tell you were just about to ovulate from these ?  Maybe I'm wrong (not unheard of !) but this is not how I've always been led to understand it.  Unless of course you also had LH and FSH tested in which case she may have been able to see higher levels of LH, indicative of LH surge ?

If you'd had a scan I would understand as they'd be able to see the developing follicle and by it's size guesstimate when ovulation would happen.

If you can get another progesterone blood test done tomorrow then I'd definitely get it....and then I would also ask the nurse exactly what your levels were last time as well....and not just be told "you're about to ovulate".

I've also no idea why the nurse is saying you have 35 day cycles....she's obviously going by the "age old" that luteal phase has to be 14 days !

If you get full flow red bleeding after 28 days then you have a 28 day cycle....as you count cd1 as the first day of full flow red bleeding and if that starts after around 3pm then the following day is cd1.

It is all so confusing I know...........but I'd def get the bloods done again tomorrow if you can and then take it from there :)

Thanks for the reply !  I went to see the doc first thing this morning and they cant see me today or tomorrow for a repeat as they are fully booked how irritating!! anyways I managed to get an appointment for this thursday which will be cd27/28 and may also get my period that day - should I still go get this blood test done?

Morning :)

Yep, I would still get your progesterone bloods done as if nothing else, will hopefully give some indication of whether your levels are rising.

If it's a level of 9nmol/l then this means no ovulation......if it's 9ng/ml then it may mean you ovulated and just tested a little early as just under the 30nmol/l they look for  (10 ng/ml is approx 30 nmol/l so 9ng/ml would be approx 27 nmol/l)

when comparing any hormone levels you need to include the unit measurement....see if you can get the measurement.

I am actually fed up of this now and just going to leave it and wait until I have my appointment with my fertility specialist on the 9th cos I have phoned the nurses at the clinic so many times that they actually getting irritated.  I just feel like I am hitting a brick wall not only is it like a time race feeling to try get pregnant before my endo comes back with a vengence but its now not even knowing if i am ovulating and if I am when am I.  I was doing the OPK (clearblue) but now feel that that is wrong as it gave me a surge on a sunday meaning i would ovulate up to 36 hours later but when I had my progesterone bloods done on the following friday they telling me that I have not ovulated!! so now im just going to take a step back and not monitor anything just focus on other things cos this is driving me nuts!

Perhaps it's the best thing to do ^hugme^

It can all become way to obsessive and you lose sight of having fun....all that lovemaking just becomes babymaking !

I'd just try to make love as often as possible....try to ignore where you are in your cycle, through away the OPKs, don't chart....just go back to having lots of spontaneous sex...and fingers crossed !

ok so went back for another progesterone yesterday (was due to have my AF but still did not arrive) and my results were 21.3 - a level greater than 20 indicates ovulation was what the nurse told me and the test result for last Friday was 9.4 which shows no ovulation she couldnt answer if it was mnol etc.

so what does this mean of my cycle how many days am i then?

if i bleed every 28 days?????
