Hi All

Wondering if anyone can help.

I have Factor V Leiden and also have recently found out that I have the MTHFR gene (another Inherited thrombophilla)... all my other tests okay... APA's, Protein C and S etc....

Im interested to know the treatment you were suggested for the MTHFR gene (pre conception care) and extra support needs during cycle and on confirmation of a pregnancy.

Hello Deb,

I was also diagnosed with heterozygous FVL and homozygous MTHFR.  There is controversy over how critical MTHFR is in recurrent miscarriage.  Some doctors only treat if homocysteine levels are elevated.  However, I have had 3 consecutive ivf miscarriages and went to Dr. Beer who treats MTHFR as a serious miscarriage factor.  Personally I believe the Drs. at St. Mary's are way behind current thinking in recurrent miscarriage and the influence of FVL and MTHFR - I think their recommendation to take Clexane on positive pg test is wrong and too late for many women.

My treatment is 40mg Clexane preconception (from about day 6 of an ivf cycle) to be taken throughout pregnancy (if I am lucky!) and also aspirin daily.  I also am on 5mg folic acid, 50mg B6 and 1mg B12 every day for the rest of my life - this is very important.  MTHFR means you do not absorb folic acid and B vits which also influent nural tube defects in the baby.  Y

This treatment cannot harm and I would advise it for any upcoming cycles.

Thanks for this info.

I have been advised for my next cycle to start 40mg heparin- I think just after ec (will need to double check) and when pregnancy is confirmed to take baby asprin too. I was told not to take heparin before ec- risk of vein getting knicked when eggs get collected etc....

Im also taking 5mg folic acid. I will ask the dr about Vit B's - Im sure he wont have an issue with me taking it.

There is a heamatolgist I may go and see at UCH- who I think also specialises in thrombosis and pregnancy- will have a chat to fertility dr when we go and see them in June.

Hiya,


I've today being diagnosed with MTHFR gene.

I already knew about my FVL, but there was a delay in the MTHFR results reaching me. I've been prescribed Arixtra and Folguard for the Factor V leiden, both of which I already know I can't get from the pharmacies where I stay..but I can always get them shipped direct from Dr Beer...just wondering if anyone knew if these two prescriptions are enough to cover the FVL and MTHFR....or if I'll need additional medication?

Hi Abby,

The treatment for FVL is usualy  Clexane (or equivalent blood thinner) plus sometimes low dose aspirin. 

The treatment for MTHFR is extra folic acid and B vits plus Clexane (or equivalent) and often low dose aspirin. I can't get Folgard in the UK so I make up my own 'cocktail' from over the counter vitamins.  This is ok as long as you are taking 5mg of folic acid, 50mg B6 and 1mg B12 daily. 

MTHFR means potentially higher homocysteine levels and a tendency to clot more (like FVL).  However, it also means you do not metabolise and absorb folic acid and B vits which can increase the risk of neural tube defects.

Dr. Beer will also usually add low dose aspirin (75mg) from day 1 of the cycle but not all doctors agree on this.

Dr. Beers protocol is to start the Clexane on CD 6 of the cycle.  You should start the folic acid and Bvits now as you need some time on them to prepare for your cycle (a good couple of months at least).

These meds. should be all you need, but obviousl double check them with your doctor always to make sure.

Thanks for the info. I've got an appointment with the IVF Doctor on Monday, and I've always found its better to go to the docs armed with all the info you need..rather than wait for them to offer to share with you...sometimes you have a long wait  ;D so I'll see what she can suggest...50mg of B6 is a lot isn't it??

Hi Abby,

The amounts of folic acid and b vits are equivalent to 2 Folgard tablets. Dr. Beer prescribes 1 or 2 Folgard tabs. I take the equivalent of 2 folgard tabs. for my homozygous MTHFR.
