Normal Day 21 Progesterone Levels - Please help a newbie!!

Hello - I'm really hoping someone out there could please offer me some advice.....

I've just had the results of my day 21 progesterone test. It was 89.

The doctor (well the receptionist) told me this was normal but I'm not so sure.

I've tried to find out what is a "normal" range for progesterone but I can only find information on low levels, not on high levels.

I'm on day cycle day 27 today and I don't believe that I'm pregnant this month.

From what I've read 89 seems really high. Can you have too high a level of progesterone when you're not pregnant?

Hi there

A level of 89 nmol/l is absolutely fine and nothing to worry about at all.  A level of 30 nmol/l or over indicates ovulation.  Sometimes a higher level may indicate more than one egg released but not always.  It just means you have had a good mature egg released and that progesterone level is good to support any possible early pregnancy until placenta takes over.....it's certainly nothing to be concerned about.

During natural cycles my levels have ranged from 48 nmol/l (lowest I've ever had and that was first cycle following IVF) and have been as high as 81 nmol/l.  I ovulate fine naturally but was prescribed 6mths of clomid 5 years ago to boost ie to release more eggs (more target practise for the sperm !).  During those 6mths I had progesterone tested twice (on the 1st and 4th cycles I took it)....my levels came back at 103 and 105 nmol/l.  Scans had shown 2 or 3 dominant follicles so this higher level, the consultant informed me, was where I released 2 or 3 eggs.

Progesterone can't be used to determine pregnancy as it varies so much month to month and between women.  Some women may have lower levels (although above 30nmol/l) and be pregnant whilst others may have high levels and not be pg.  Only a pregnancy test and/or HCG blood test can confirm pregnancy....progesterone will only indicate whether you've ovulated.

Progesterone peaks at 7dpo (7 days past ovulation) so this is ideally when it should be tested.  Having tested on cd21 assumes ovulation on cd14.

Hopefully that's put your mind at rest....the receptionist is correct (as she would've been reading your GPs notes!)....and yes, 89 nmol/l is completely normal, nothing to worry about.

Thanks very much for taking the time to reply. You've really set my mind at rest with this information.

I've had a problem in the past (unrelated to pregnancy) with just accepting blood tests results as 'normal' because the receptionist said so and which later turned out not to be quite the case, so I'm a little wary! In fact as a result of this previous issue, I asked for the actual levels of progesterone this time, usually to be told they were normal would be enough.

anything above 50 is deemed as healthy and very fertile.  Mine came in at 24, so I'm in the low range and i'm having treatment. 

So keep going - you're ovulating nicely!

Hi there "Nervousme" and welcome to FF :)

Any progesterone level of 30 nmol/l or over indicates ovulation of a mature egg (unfortunately progesterone can indicate quality of egg or ovarian reserve).

Testing on cycle day (cd) 21 assumes ovulation on cd14....progesterone peaks at 7 days past ovulation (dpo) so this is ideally when it should be done. If ovulate earlier or later than cd14 then should try and get tested accordingly eg if you ovulate on cd16 then get progesterone tested on cd23 etc.

If you had a progesterone level of 24 nmol/l on cd21 then this may mean the egg wasn't quite mature enough (they like follicle to be minimum of 18mm before it pops to release egg).....or it could mean that you were tested on the wrong day. A level of 24 nmol/l would be seen as "borderline" ovulation.....you may just have ovulated a bit later than cd14 so progesterone hadn't quite peaked (as not 7dpo).

Have you had any other hormone blood tests eg FSH, LH, Oestradiol (E2), AMH, Prolactin etc ? How long have you been ttc and have you already started having fertility treatment ?
